JOB DESCRIPTION SUMMARY
Looking for a Registered Nurse with Long term care experience. The registered nurse will be mainly performing baseline wellness assessments for a long term care facility.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
- Patient Care
- Ability to complete an initial assessment to determine any evidence of abuse, neglect, wounds, or ulcers.
- Provides a complete physical and full body skin assessment as well history of current and previous illness(es).
- Documents completes assessment based on wellness assessment checklist provided.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Report any evidence or cases of abuse, neglect, ulcers, wounds or red flags within facility..
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Prepares clinical notes and progress notes and updates the Director of Nursing (DON) when necessary.
- Communicates with the DON regarding the patient’s needs and report any changes in the patient’s condition.
- Participates in in-service programs and supervises and teaches other nursing personnel and aides.
- Create an environment for open communication.
- Graduate of an accredited school of nursing.
- Previous home care/long term care experience is a plus.
- Current licensure in Maryland or compact license for multiple states (with Maryland included),
- CPR certification, and valid driver’s license.
- Must be able to pass a state background check.
- Excellent observation, verbal and written communication skills.
